At Find My Lab Testing (FMLT), your health and privacy are our top priorities. We are committed to maintaining the confidentiality of your Protected Health Information (PHI) as required under the Health Insurance Portability and Accountability Act of 1996 (HIPAA) and its implementing regulations. This Notice outlines your rights and our obligations with regard to your health information.

Understanding Your Health Information

Each time you visit a lab or utilize lab testing services through FMLT, a record of your interaction is created. This record typically includes:

  • Your symptoms
  • Lab test results
  • Demographics
  • Treatment-related communications
  • Payment details (if applicable)

This information is collectively referred to as your Protected Health Information (PHI). PHI is any individually identifiable information related to your past, present, or future physical or mental health.

How We May Use and Disclose Your PHI Without Authorization

We may use or disclose your PHI without your explicit consent for the following purposes:

  1. Treatment:- PHI may be shared with physicians or labs as necessary to facilitate testing, order review, or result interpretation.
  2. Payment:- We may use your PHI to process payments for lab services you have purchased out-of-pocket, as FMLT does not process insurance claims.
  3. Health Care Operations:- PHI may be used for quality improvement, compliance review, and internal administration. These functions may be managed by our HIPAA-compliant service providers.
  4. Legal Requirements:- We may disclose PHI as required by law, including subpoenas, court orders, or public health disclosures (e.g., to prevent disease or abuse).
Your Privacy Rights

You have the right to:

  • Receive a copy of this Notice at any time.
  • Request restrictions on how your PHI is used or disclosed.
  • Access and obtain a copy of your test results and related PHI via your secure FMLT portal.
  • Request amendments to your PHI if you believe it is inaccurate.
  • Receive a record of certain disclosures made without your authorization.
  • Request confidential communications (e.g., use of alternate contact details).
  • Revoke authorizations you’ve previously provided, except where actions have already been taken.
Security of Your Health Information

FMLT ensures all communications and storage of PHI meet stringent HIPAA standards. Test results and lab orders are never sent via email. Instead, customers can access them securely by logging into their dedicated patient portal, where lab orders and results are available for download.

Due notifications are sent at each step of the process, including order placement, lab order availability, and results release.

Use of Business Associates

FMLT may use HIPAA-compliant service providers, such as physician networks and certified laboratories, to support your services. All vendors must sign Business Associate Agreements (BAAs) to ensure your data is handled in compliance with federal law.

We Will Never Sell Your Information

FMLT does not sell or rent your PHI to third parties. Your information will not be used for marketing or promotional purposes without your explicit authorization.

What If There Is a Breach?

In the unlikely event of a breach of your unsecured PHI, you will be notified promptly, as required by HIPAA.
